Thomas W. Puckett Sr. January 26, 1955-November 14, 2017 Mr. Thomas W. Puckett Sr. , 62, of La Porte, Indiana, passed away Tuesday November 14, 2017 at his home in La Porte, IN. He was born January 26, 1955 in Hawesville, Kentucky, the son of Woodrow Puckett and Mary (Lawson) Puckett. He is survived by his life companion, Laura Severs; son, Thomas W. Puckett Jr.; step son, Tim Michaels; grandson, Thomas W. Puckett III; sister, Nelda Johnson; brother Charles Puckett and many nieces and nephews.
