Donna Marie Forst, 69, of Westville, Indiana, passed away Friday November 20, 2020 at Brentwood Assisted Living in LaPorte.

She was born November 22, 1950 in Chicago, Illinois, the daughter of Olivio and Phyllis (Benadello) Carlmango.

On June 23, 1972, Phyllis married Barry Lee Forst, who survives. Together they lived in Portage for 30 years, moved to Valparaiso for a short time, then Westville until her passing. Phyllis was the manager at Jewel Supermarket. She was a baptized and confirmed Catholic and really loved spending time with her husband, going out and attending plays together.

Along with her husband, Phyllis is also survived by her two sons, Brian Forst of Westville, IN, Kris Forst of Moracco, IN; sister, Karen (Joe) Gatto of Tampa, FL; grandchild, Leo Forst.

Arrangements are being entrusted to Haverstock Funeral Home and Cremation Services, 602 Maple Ave., La Porte, IN. Cremation has been selected.
